Welcome to the HPU Athletic Training Online Participation Forms Portal

This new automated process will allow us to be more efficient in handling form submission and review to get you ready to participate in team activities upon arrival to the HPU campus. If you have any problems, please contact the athletic training staff.

To access the online forms, use the button above (click here). You will be taken to the “Welcome to Parent Portal Screen”.

Prior to completing the forms create an account using your HPU email address, you will also need to know your HPU student ID number. This allows you to view the status of your forms or download copies of the completed forms. You will be taken to a page listing the forms to be completed. Please fill out all required spaces and select appropriate answers.

New student-athletes must complete all forms. Returning student-athletes will need to complete everything but the physical and sickle cell results. Checklists may be found below.

Click on the form name and fill out the information requested make sure to fill out first name, last name, student id, school attending is at the top of the document. To sign select if you are over 18, or a parent/ guardian sign with you. You may be asked to sign with your mouse, click on the box, hold mouse down and sign the form. Please put in a notification email: preferably your HPU email. Then once the form is submitted you will receive a confirmation email once the form has been reviewed by the HPU AT staff.

Please note that the pre-participation physical will need to be turned separately to the HPU AT staff. New and transfer student-athletes are the only ones that need physicals. If a student-athlete left HPU for another school then transferred back they would also need a new physical.

A medical check-in, concussion education, and C3Logix concussion baseline testing will occur in person on campus prior to sport participation.

Required Forms Checklist – New/ Transfer Student-Athletes:

1. Pre-Participation Physical Exam: must be completed by MD, PA, NP. NCAA does not allow DC (chiropractor) to complete a physical. Physical must be dated within 6 months of school start date.
2. Sickle Cell Results: Every student-athlete must provide sickle cell trait results due to updated NCAA legislation. After the 2022/23 school year, only new/ transfer student-athletes will be required to complete this.

All forms (athletic training and compliance), concussion baseline testing and education, and a medical check in are required before you can participate in any sport related activity (ex: weight lifting, conditioning, practice).
